This assistant is trying her hardest, but she fundamentally misunderstands a lot of things. Some people are just like that: they're very bright in some ways, and completely inept in others. Maybe you have a coworker or two who fits that description (or maybe you are that person!). These people can sometimes be so-called "personality hires." That's the person who is hired just because they're fun to be around and have a good energy about them, which can be morale-boosting in a work environment. This kind of hiring can also drive other more qualified employees up the wall, though.
This person seems like she's having a difficult time understanding some rather basic things in her medical profession, though. As u/lucid_aurora explained in their story about their "Kevina" coworker, this woman was confused about how staplers work, and she was totally baffled by one paperwork process that seems otherwise straightforward.
