While we accepted the fact that Tony Stark gave his life in order to defeat Thanos, This is not an "absolute point" in time. meaning, this event could have played differently across the multiverse. In some universes Tony Stark lost, In some he won, In some he wasn't even involved and in some - He survived.

Adamantium Armor
In one of these universes, Tony was digging deep into the earth, trying to find a metal that could compete with Thanos's Vibranium shattering sword. Eventually, Tony found this new type of metal, which he named "Adamantium", Not knowing he actually stumbled upon a sleeping celestial.

The Mark XXXI
Using this new Celestial metal, Tony builds himself a new suit - The Mark XXXI (Model 31, appearing in Iron Man Volume 4 #11). This metal gave Tony the edge over the mad titan, and once he grabbed the stones out of the Infinity Glove, This metal also absorbed a lot of the energy of the snap, allowing Tony to survive the cosmic event.

As the Adamantium absorbed the immense energy of the snap, It melted and fused with Tony's body, and so did the Infinity stones. Tony was left trapped inside the impenetrable suit, that now also bestowed him with the powers of a god.

The Ultimate Iron Man
At first, Tony used his powers to become the greatest hero the world had ever seen. However, being trapped inside the suit slowly drove him mad (No, he couldn't use the stones to remove it, the Adamantium is immune to the powers of the stones). He was still good, but he felt increasingly distant from the rest of humanity.

To make things worse, The Avengers were not able to return the stones to the exact moment they took it from, thus creating branches in reality and creating several "Nexus events", dooming several universes

With the help of the Watcher, Different versions of superheroes arrived from all affected timelines to reclaim their stolen Infinity stones. A multiversal war began but no one was able to defeat the ultimate Iron Man. They tried to explain to him that he was dooming all their universes but he was unable to help them.

Eventually Tony understands that there is no way to undo what he has done, he can only try to minimize the damage. With the power of the stones he starts exploring the multiverse and comes across the sacred timeline - one universe that was separated from the rest - Universe 616 (yes, it's not 199999 anymore).

Doctor Doom
Tony enters universe 616, finds this universe's version of Earth, and goes into hiding in a little known country named "Latveria" where he does what he can to help the locals while keeping a low profile. Riddled with guilt over the doomed universes he left behind, Tony presents himself as "Doctor Doom".

However, After the events of Loki, As the sacred timeline begins to break, Tony starts building an army of Doom bots to help earth 616 survive the incoming multiversal war.
